An emergency department running like clockwork
FEBRUARY 2020 - The Antony Private Hospital in the South of Paris, has the largest private emergency department in France. 15 emergency physicians welcome 70,000 visits a year, with peaks of 280 patients on certain days. The practitioners organize a presence of up to 6 doctors on the front line at certain times, following the attendance curve, which is roughly similar in all hospitals.
They work in pairs, mainly with the nurses, and divide up the roles between them: reception and assessment, orientation, management of traumatology in a short circuit approach, and medicine and difficult cases in a long circuit approach. The success of the model also lies in the cooperation with the imaging platform and the analysis laboratory, which are committed to quick deadlines 24/24.
Finally, emergency physicians benefit from a post-emergency sector which allows them to wait up to 48 hours before referring patients to another department, to another establishment or to their home. 7% of emergency patients will ultimately be hospitalized. The severity profile of the patients and their pathologies are identical to those encountered by our colleagues at the public hospital.
An educational display
The waiting time before the first check as well as the total transit time are calculated continuously on the basis of the previous 4 hours and displayed on site and on the website: it averages 15 to 20 min. . It is neither a promise nor a guarantee, it follows a logic of transparency. This indicator is above all a marker of the overall quality of the organization, and of the attention paid to this subject, which leads to more respectful behaviour from the patients.
Solutions to relieve congestion in the emergency department
Since September 2019 a reorientation of general medicine patients after evaluation is experienced. They are addressed to a practitioner who consults without appointment 3 days a week and at weekends, within the medical house which is hosted on site, thus benefiting from the proximity of the imaging and biology platforms. For several years now, a fast circuit for general practitioners has been implemented. Identified patients thus have access - without an appointment - to the technical platforms, and general practitioners receive the results during the day, enabling them to finalise their care from start to finish, without inappropriate use of the emergency department. The key is to succeed in aligning all energies towards the same goal: patient care.
